Redemption Form for Taxi Drivers |

Travel Vouchers

Travel vouchers can be claimed instead of a bus pass by some residents. Travel vouchers can only be used as part or full payment for taxi fares.

Who can apply?

1. Residents who have been awarded the high rate of the mobility or care element of Disability Living Allowance OR the high rate Attendance Allowance PLUS housing benefit or council tax benefit. Proof of your benefit will be required.
2. Residents aged 85 or over. Proof of your date of birth will be required.
3. Residents who have had a learning disability that started in childhood (please contact us for full details). Proof of your disability will be required.
4. Residents who have been awarded the War Pensioners Mobility Supplement PLUS housing benefit or council tax benefit. Proof of your benefit may be required.

If eligible you will receive travel vouchers in £1 denominations to the value of £30, free of charge, with an expiry date of 12 months.

Details of taxi companies who will accept travel vouchers will be supplied with the terms and conditions leaflet issued with the travel vouchers.

How to apply

An application form is available in two formats, both can be completed on your pc but must be printed and signed before returning to DDC.

Terms and Conditions of the Scheme

Take your completed and signed application form to any of the @ Your Service Offices listed with proof of your age or disability. If you need guidance on proof required, please phone or email for details.

Please indicate which @ Your Service Office you would like to collect your vouchers from.

Please provide 1 recent passport size and style photo with your application.
Please write your name and date of birth on the back of the photo and enclose it in an envelope attached to the form.

 

Lost or Stolen Vouchers

If your vouchers are stolen you must report the theft to the Police. You must then complete a form and new vouchers will be posted to you. If your original vouchers are found, please return them to us for cancellation as they will not be valid for use
