Application Forms
The links below will take you into the most commonly used forms
under the Gambling Act 2005:
1. Application
for a premises licence.
- This form covers premises licences for casinos, tracks, betting
premises, bingo, adult gaming centres and family entertainment
centres wishing to make category C machines available for use

Application to transfer a premises licence
- Required where an existing operator wishes to transfer the
premises licence to another operator.
3. Application
to vary a premises licence
- Required where an operator wishes to change the layout of a
premises, the hours open to the public or activities carried out on
the premises.
4.
Notification of 2 or less gaming machines in a premises
where alcohol is sold.
- This form allows the holder of a premises licence to notify
that not more than 2 gaming machines of category C or D are
available for use on the premises. Each transfer of the
premises licence will require a fresh notification to be made.
5.
Application for 3 or more gaming machines in a premises
where alcohol is sold.
- This allows the holder of a premises licence to make 3 or more
gaming machines of category C or D available for use. This
permit is transferable and incurrs an annual fee.
6.
Application for a Club Machine Permit or a Club Gaming
Permit.
- Required to allow the provision of gaming and gaming machines
in members clubs and commercial clubs. Restrictions apply on
the type of gaming according to premises. Please contact the
Licensing Team if clarification is required.
7. Application for a personal licence
- Both Personal Management Licences and Personal Functional
Licences are available from the Gambling Commission.

8. Application for an operators
licence
- Applications for the various remote and non-remote operators
licences are available from the Gambling Commission.

9. Application for a
small society lottery
- Required where a charitable or small society wishes to hold a
lottery. Guidance notes are also
available.
